Sarasota, FL- 36667871 ASAP Start! Mon - Fri., 8am - 5pm, 40 hours
per week DIRECT HIRE RN 40 hours per week, working days, some
weekends roughly. Will discuss further in interview. Pay is on a
point system, overall hours will vary based on cases and types of
visits. Salary is based on location and skill level Pay rate:
$37.50 per hour Must be a US Citizen Duties: Job Summary: The Home
Health RN is responsible for identifying and coordinating patient
and family needs, and for providing palliative, preventative, and
rehabilitative care to the patient/family unit. The RN utilizes
assessment, intervention, and teaching skills to maximize patient
comfort and enhance the quality of life for the patient/family
unit. This role follows all home care regulatory and compliance
requirements and meets all Joint Commission standards. Job
Responsibilities: -Provides ongoing assessments of the physical
needs of the patient/family/PCC and initiates appropriate
interventions or referrals based on the patient's plan of care.
-Provides accurate, timely, and appropriate documentation of
patient/family services in accordance with HomeCare policies and
procedures. -Serves as the case manager to ensure continuity of
care and submits all required documentation promptly to meet
regulatory and compliance requirements. -Evaluates services
provided by Home Health Aides every two weeks through on-site
supervisory visits. -Participates in providing 24/7 patient care
coverage. -Follows all State, Federal, and Joint Commission
regulations and standards. -Educates employees on regulatory
standards and home health service implementation. -Reviews and
coordinates certification and recertification documentation for
skilled patients to ensure completeness and accuracy. -Assists in
orienting new HomeCare staff members. -Evaluates clinical records
and prepares required forms for home health billing to the
intermediary. -Assists in developing quality indicators and
implementing the home health quality assurance program.
-Participates in survey site visits for home health. -Participates
in Home Care meetings, assists with departmental planning, and
supports adherence to the annual budget. -Performs other duties as
requested by the Home Care Manager. Skills: Required Skills &
Experience: -Minimum of one (1) year of current active nursing
experience. -Passing grade on a pre-employment computer literacy
evaluation. -For field-based positions: reliable transportation and
ability to perform responsibilities in a timely manner; proof of
valid automobile insurance. Preferred Skills & Experience: -One (1)
year in an acute care setting. -One (1) year of home health
experience with understanding of State, Federal, and Joint
Commission Home Health Agency Regulatory Standards. -Intravenous
therapy training and experience. -Experience caring for terminal
patients and others with life-threatening illnesses. -The ability
to type 25 wpm. Education: Required Education: -ASN or ADN.
Preferred Education: -N/A Required Certifications & Licensure:
-Valid Florida license as a Registered Nurse. -Valid Florida State
Driver's License. -Valid BLS certification from the American Red
Cross or American Heart Association. -Meet continuing education
requirements for licensure. Preferred Certifications & Licensure:
