• Collaborates with nursing, case managers and physicians, meeting with discharging patients prior to discharge to discuss care transition from the hospital to home.
• Provides both in-hospital and post hospital transition of care discharge communication and understanding of care, responsibilities, medication management and post-acute follow up with PCP and Specialist to the patient.
• Identifies opportunities for improvement in the patient's transition of care from the hospital to home and facilitates patient discharge support functions with nursing, case management and hospitalist.
• The Transition of Care Case Management role is independent of the Case Management role and focuses/prioritizes on the patient's transition of care from the hospital to home. -Education: Graduate of an accredited university/school of nursing. Bachelors in Nursing or health related field preferred. -Experience: 3-5 years acute care experience, Preferred Case Management Experience-Discharge planning/post hospital follow-up management. Licenses/Certificates: Current Florida RN license -INDPR
