With established faith-based principles and dedication, we've been able to see thousands of individuals and families regain their independence over the past 30 years through our numerous programs.ABOUT THIS ROLE:The primary responsibilities for this role is to provide case management to the residents in Justin's Place in LaBelle. The Sr. Case Manager will assist the resident in achieving self-sufficiency through advocacy; setting goals, accessing community resources and support services. Sr. Case Manager will work one-on-one with residents through case management to develop plans/goals and evaluate progress.WHAT YOU'LL BE DOING:Provides individualized case management for assigned case load.Refers clients to other agencies based on their needs and in accordance with their program plan.Work with outside agencies to ensure residents are receiving the medical care and mental health care necessary for success.Facilitates morning classes in accordance with program curriculum.Works in collaboration with the team in providing and maintaining spiritual meetings, AA meetings, church, and events that help promote the values, principles, and beliefs of SMH.Helps coordinate transportation needs for clients: includes printing and overseeing daily list with client appointments to ensure that transportation to and from approved locations is provided.Help prepare residents for transition to Wolfe Apartments where they will be in more community style setting and obtaining employment.Answer questions of residents and directs them to appropriate staff to resolve issues: also directs residents to appropriate services in the community.Observes clients receiving medication and documents system case notes when appropriate.Performs random drug screens and breathalyzers as needed on residents.Helps oversee daily recovery team and curriculum schedules of Justin's Place, LaBelle (JPL).Mentors and trains members of the Recovery Team and Staff on best practices of addiction treatment.Plans, coordinates and facilitates recovery program classes from time to time to assist clients in obtaining skills that will enable them to cope with their environment.Manages vehicle maintenance service, logs and quarterly reports.RequirementsWHAT WE'RE LOOKING FOR IN YOU:A minimum of four (4) years related experience in social services, addiction treatment, counseling or related field. Experience working in a faith-based non-profit a plus.Bachelor's degree in social services, counseling or related field required. State certification or licensure in addiction treatment will satisfy educational requirement.Demonstrated knowledge of substance abuse and co-occurring disorders.Thorough knowledge of individual and group counseling techniques and the ability to effectively facilitate groups.Understanding of motivational interviewing, brief interventions, counseling, referral, and case management process.A demonstrated ability to work with individuals of complex needs and diverse populations.Ability to work collaboratively as part of a team.Must be able to stand and/or sit for a minimum of 4 hours at a time and life at least 25 pounds. Must be able to bend or kneel to the ground.Willingness to adjust schedule, tasks, and priorities when necessary to meet business needs.
